Accreditation Scope · Official accreditation-program comparison

NCQA and URAC utilization-management scopes differ

NCQA publicly describes an organization-level framework for fair, timely utilization decisions using objective, evidence-based criteria. URAC publicly describes five functional modules and U.S. eligibility conditions. Program fit begins with the legal entity and functions performed, then requires the current licensed standards and official accreditation record.

Define the applicant and functions before comparing programs

Start with the legal organization, clients and populations served, jurisdictions, medical or behavioral-health scope, utilization functions actually performed, delegated and retained work, clinical-decision authority, appeals, locations, subcontractors, and requested accreditation purpose. NCQA's public page describes utilization-management organizations evaluating requested services on behalf of clients; URAC's page states U.S. operating and medical-necessity, peer-review, and appeal conditions for its Health Utilization Management program. [1] [2]

That inventory prevents a brand-level comparison. A health plan, utilization-management company, delegated service provider, clinical-review organization, and software vendor can occupy different positions even when their workflows connect. The accreditation object, contracted service, software component, and individual authorization decision should remain separate records. [1] [2]

Compare public program structures without inventing equivalence

NCQA's overview presents a quality-improvement framework focused on fair and timely evaluations, objective evidence-based criteria, relevant clinical information, qualified decision makers, and state alignment. It also identifies a 2027 survey-readiness package for surveys beginning July 1, 2027 through June 30, 2028. The page does not provide a public module crosswalk to URAC or enough detail to infer equivalence. [1]

URAC publicly exposes five functional modules. Pre-Review Screening covers the start of prior authorization through paper, automated, or semi-automated intake; the other modules distinguish initial clinical review, medical-necessity decisions, criteria development, and comprehensive review including appeals as applicable. Record the exact modules sought or held instead of treating the HUM seal as one undifferentiated function. [2]

Use licensed standards and official status for the decision

A program-fit record should preserve the accreditor, program and edition, survey period, applicant entity, functions and modules, standards entitlement, jurisdictions, delegation contracts, gap assessment, evidence owners, interpretation questions, application state, survey findings, corrective work, final status, effective period, and official directory or certificate evidence. Marketing pages can frame the comparison but cannot replace the controlling standards or accreditation record. [1] [2]

For a delegated arrangement, map each authorization stage to the organization performing it and the evidence retained: intake, completeness, criteria selection, initial review, peer review, determination, notice, reconsideration or appeal, oversight, and exception. Accreditation evidence for one entity or function does not automatically travel to an affiliate, subcontractor, software product, different module, or work retained by the delegating plan. [1] [2]

Test one delegated request across the claimed boundary

A representative evaluation should route one request through automated intake, an incomplete clinical record, initial clinical review, peer review, an adverse determination, notice, and appeal. Reviewers should identify the organization and authority at every step, map the claimed NCQA or URAC scope, verify the applicable program edition and official status, and expose any function that falls outside the claim or remains with the health plan. [1] [2]

The reviewed public pages support only the program descriptions stated here. They do not decide which accreditation a specific organization needs, establish parity between programs, grant accreditation, validate delegation, prove an authorization was correct, or establish compliance with state, federal, contractual, clinical, timeliness, notice, privacy, or appeal requirements. Qualified accreditation, clinical, operational, regulatory, and legal review remains necessary. [1] [2]

Limitations and unknowns

  • The complete licensed standards, survey tools, interpretation guidance, contracts, and accreditation files were not reviewed.
  • No applicant, delegation, module mapping, authorization case, clinical decision, survey, finding, accreditation status, or outcome was tested.
  • Program fit and applicable state, federal, contractual, clinical, notice, privacy, and appeal requirements remain organization-specific.

Enterprise buyer test

Translate this change into the exact population, record type, workflow stage, decision owner, effective date, and evidence that could be affected. Ask current or prospective providers to demonstrate the named workflow with representative data and an exception—not a polished feature tour. Record what official documentation establishes, what a provider states, what the team observes, and what remains unresolved.

A defensible review also identifies the dependency outside the product. Authority interpretation, policy configuration, data quality, integrations, human judgment, approval rights, release governance, training, and retained evidence may remain customer or service responsibilities. The evaluation should preserve those boundaries instead of treating a technology claim as the complete operating model.

What we will watch next

Prior Auth Monitor will watch the named source and affected market records for later evidence that changes status, scope, availability, implementation timing, workflow consequence, or the limits of the initial report. A later announcement does not silently overwrite this dated account; the change ledger preserves the sequence.
